PERMISSION AND RELEASE:

I _______________________(print name), in participating in the 2009 New York City Public Ultimate League (NYCPUL) at McCarren Park, understand that danger exists with respect to Ultimate frisbee (the “Activities”) due to many factors, including but not limited to the actions or inactions of others participating, the weather, field, facility, observers or other prevalent conditions. I also understand the nature of the Activities, and acknowledge, represent, and warrant that I am qualified and physically able to fully participate in such Activities and am participating at my own risk.

I release and discharge the NYCPUL, its administrators, organizers or volunteers from any and all claims, actions or liability arising out of or in connection with my participation in the League. This release includes any loss, damage, or liability, or other fault that may result in personal injury, death, or property damage during or in connection with the above League or .Activities This release will be construed according to the law of New York State. This Permission and Release shall inure to the benefit of licensees and assigns of the New York City Public Ultimate League, and shall be binding upon the participant, , his/her heirs, estate, personal representatives and assigns.

* DISCLAIMER: All players who register for this league agree by this general waiver, release of liability, assumption of risk, consent and indemnity agreement. You understand that Ultimate is an activity that involves physical risk with real possibility of injury. You agree to release, discharge, and to hold harmless the league organizers, and will provide a signed waiver to that effect before taking the field. Further, be advised that we are collecting these fees primarily to pay for an extensive sports permit with the NYC Parks Department, that has confirmed the permit to run from April 6th through October 9th, from 6-10PM on Monday evenings. If for any reason that permit is cancelled, refunds will be issued (minus Paypal processing fees). The organizers reserve the right to cancel registration at any point. *In participating in the player poll for league format, you agree to abide by the determination of the poll, and, if you do not agree with that determination, fees are only refundable at the discretion of the organizers. It's a great deal for a tremendous number of hours of high quality ultimate on one of the best athletic fields in the city. If there is a question about whether or not that will be good enough for you, please do not register.
